The Forest Warren Human Services Drug and Alcohol Prevention Program is conducting a survey to learn more about drug and alcohol use and adult attitudes about use in our two-county area. The survey only takes about 10 minutes of your time and it is completely anonymous – none of the questions ask for your name or any identifying information. The survey results will help us target our prevention efforts in order to focus on issues that our communities are concerned about. We would very much appreciate your help in completing the survey.
